Please be aware, because of state law, we cannot address anonymous complaints.
Please include any handouts, written material(s) received during your office visit, receipts or advertisements that are pertinent to your complaint.
If applicable, please provide the name of any other individual who has information supporting your complaint and/or was a witness to any incident relative to your complaint.
