Application & Renewal Forms
Every person practicing chiropractic in the State of Louisiana must meet all requirements for a license or license renewal and be currently licensed by the Louisiana State Board of Chiropractic Examiners with an initial license and subsequent annual license renewals.
*PLEASE NOTE: Beginning July 1, 2011, each applicant for licensure shall hold a Baccalaureate Degree from a college or university approved by an accrediting agency of the United States Department of Education. (LSA R.S. 37:2805.B.l.d)
